OKcupid also offers this answer:
2.5. Why's so-and-so (whom I match!) not in my match results?
Every time you click for a match, we combine the best 50 people from your last match and a new set of random people... anywhere from 200 to 10,000. We then recalculate matches on all these people and show you the best. This means your best matches stick around -- as long as they remain good matches -- while allowing newly discovered matches to join the list.
If someone doesn't show up, then either (a) they haven't been found for you yet, or (b) they don't fulfull your match restrictions, as set on your settings page.
This system is subject to major change, and, at the very least, improvements. As we improve OkCupid's speed, we'll increase the batch sizes to find good matches more quickly.
One last point: matches are cached every ten minutes for you. Re-clicking "world matches" or "local matches" multiple times in a short period will show the same people.

It's a good thing I have other people to read the FAQ for me.
